Visa^® Application Instructions

(Your 1st time applying for a credit at E&G or have not applied for a credit in 3 years or more)

1. Download the "Full Application" and open the application on your computer.
   
   
2. You may either print out and hand fill in the required information or simply click on each field and type in the information.
   
   
3. Once the application is complete print it out and sign the areas indicated on the application.
   
   
4. Download an "Employer Verification Form" and type in the required information (indicated in blue), then sign the form where required.
   
   
5. Return the completed application, employer verification form, a copy of a valid photo ID (ex. Valid license {not expired})and your most recent pay stub to E&G EFCU (In person, via mail or fax (914) 946-2910).

Visa^® Mini-Application Instructions

(Members who have applied for a credit at E&G in the past 3 years)

1. Download the "Visa^® Mini-Application" and open the application on your computer.
   
   
2. You may either print out and hand fill in the required information or simply click on each field and type in the information.
   
   
3. Once the application is complete, print it out and sign the areas indicated on the application.
   
   
4. Return the completed application, a copy of a valid photo ID (ex. Valid license {not expired})and your most recent pay stub to E&G EFCU (In person, via mail or fax (914) 946-2910).
